Bringing The HCD Model to Wales: How It Works

"So how does it actually work?"
It's the question we hear most, so here's The HCD Model, step by step.
1. You choose your team
You decide who supports you. That might be a family member, a friend, a neighbour — someone who already knows you, your routines, and what a good day looks like for you. If you don't have someone in mind, we help you recruit locally: local people supporting local people.
2. We make it official
We employ your chosen Personal Assistants directly, on permanent contracts. That means holiday pay, sick pay, pension, and the security of proper employment — not zero-hours agency arrangements. We handle payroll, insurance, HR, and everything that comes with being a responsible employer, so your family doesn't have to.
3. We train to your needs
Every PA is trained to CIW standards, with training tailored to the specific needs of the person they support — not a generic induction. Where clinical tasks are involved, our nurses train, assess, and delegate competencies formally, and remain accountable for that clinical oversight.
4. Our nurses stay involved
This is nurse-led care. Our clinical team oversees every package on an ongoing basis: reviewing support plans, monitoring health needs, and adjusting as things change. You get the warmth of a support team you chose, with clinical rigour behind it.
5. You stay in control
Rotas, routines, how your days look — built around your answers, and revisited whenever you want to change something. Personalisation isn't a feature of the model; it is the model.
The HCD Model has supported people across England for over 20 years through HomeCareDirect. HomeCare Direct Wales brings that same approach to Wales, regulated by CIW and delivered by local teams.
